Thank you for choosing Great Alchemy. We are committed to your health and wellness. Please understand that payment for services is a normal part of your treatment and care. We believe that a good doctor-patient relationship is based on honesty, transparency, clear communications, and up-to-date patient info. Our staff will make every effort to clarify any questions you have concerning your balance.

For your convenience, we have addressed the most frequently asked questions below. If you need additional information about any of these policies, please ask to speak with our Business Manager, Jessica Brock.

Financial Responsibility

You are financially responsible for the payment of all tests and services rendered by Great Alchemy.

Payment

Unless previously arranged, payment is due in full upon treatment. For your convenience, we accept payment by cash, check, VISA, MasterCard, and Discover.

Missed Appointments

A $50 fee will be charged for all missed appointments. To avoid being charged the $50 fee, you must notify our office at least 24 business hours (one full business day) prior to your appointment. Three (3) non-cancelled missed appointments is our limit. After your third non-cancelled missed appointment, we reserve the right to request that you seek the services of other health & wellness professionals.

Service Charges

An additional billing fee of $25.00 will be added to any statement billed after 75 days of service.

Chart Copying

A service fee of $25 to $50 will be charged for copying your chart. The exact cost depends on size of chart and how long it takes to sort and copy.

Delinquent Accounts

Unless other arrangements have been made, an account is considered past due after 30 days. Unpaid accounts beyond 120 days are considered delinquent and will be forwarded to our collection agency. These accounts will also be charged an additional $25 service fee.

Legal Fees

Any client sent to collections will be responsible for all collection fees. If a client is taken to small claims court, the client will be responsible for all fees/charges.

Prescriptions and Refills

Our policy is for the patient to call their pharmacy and ask them to fax the request for your medication to 303-444-0838. Requests are handled within 48 business hours (two business days). Also, processing times may vary depending upon your pharmacy.

Phone Calls to Doctors

Our physicians do not practice medicine over the phone. If you have an urgent need to speak with your doctor, we will give you the soonest possible appointment.

After-hours Emergencies

If you need immediate medical care outside our regular hours, please go to the nearest emergency room or urgent care center.

Calling your doctor after hours will result in additional charges.
